Instructions for DMEs, Physicians and Medical Facilities Prescription Device

The Passy-Muir Valve is a prescription device and requires an order from a medical doctor. The prescription you receive should be maintained in the patient's file. Do not send a prescription to Passy-Muir Inc. unless requested.

To Place An Order

Passy-Muir Inc. accepts orders from hospitals, durable medical equipment companies, doctors' offices, nursing homes and pharmacies. Orders may be placed by phone, fax, or mail.

Each order must include the following information:

  • Customer ID # when known
  • Billing address and phone number(We cannot mail directly to a patient or P.O. Box)
  • Shipping address, phone number and contact name
  • Product Number, Description, and Quantity (Each valve sold separately.)
  • Facility Type and Department requesting the valve
  • Shipping Requirements (Standard or Overnight)

Refund Policy

Passy-Muir Inc. does not accept returns or exchanges.

Shipping

Orders are sent via UPS. Overnight delivery and, in some areas, Saturday Overnight delivery are available at an additional cost. Please speak with a customer service representative to learn about your options.

Billing

Billing occurs at the time of shipment. Payment is due 15 days upon receipt. Payment can be made via check, money order, Visa or MasterCard. An account with Passy-Muir Inc. is established when placing your order. Pre-Approved credit is not required.
Reimbursement

The Passy-Muir Tracheostomy & Ventilator Swallowing and Speaking Valves are Medicare & Medicaid reimbursable utilizing HCPCS prosthetic Code L8501. Many insurance companies follow Medicare guidelines and usually provide reimbursement. Many states have services available for children under 18 and may reimburse for the Passy-Muir Valve. California Children's Services cover the Passy-Muir Valve under code #7549. MediCal covers the Passy-Muir Valve under Misc. Supply Code #9981K with modifier ZZ (include a copy of invoice with re-order number). Department of Defense Cage Code OU182.

Instructions for patients in the US or Canada

The Passy-Muir® Valve is a prescription device requiring a doctor's order to purchase.

There are several ways to order a Passy-Muir Valve:

Ordering Through a Home Respiratory Equipment Company

With a prescription, your home equipment company can order the Passy-Muir Valve for you and bill your insurance company. If the equipment company is unfamiliar with the Passy-Muir Valves please provide them with our phone number and we will contact them to assist them with ordering the valve. Please be sure your prescription includes the description of the valve your doctor recommends.

Ordering Through a Physician

Your physician can order the Passy-Muir Valve for you. However, many physicians are unable to directly bill your insurance company and may ask you for payment in advance. You would then submit your receipts to your insurance company for reimbursement.

Through a Hospital

If the person utilizing or requiring the Passy-Muir Valve is in a medical facility, the facility can order the Passy-Muir Valve. The hospital will bill the insurance company along with the other hospital costs.

The Passy-Muir Valve cannot be purchased directly by non-medical personnel or companies. It is important to know which Passy-Muir Valve product to order as it is a prescription product and is not returnable.
The Passy-Muir Valves are Medicare and Medicaid reimbursable using the prosthetic billing code L8501. Many states children's services also provide reimbursement. Because the Passy-Muir Valves are Medicare and Medicaid reimbursable, many insurance companies will reimburse following these government guidelines.

Instructions for customers located outside of the U.S. or Canada

Passy-Muir Inc. has distributors in over 40 countries. For countries where we do not currently have a distributor, we can accept orders from healthcare facilities, medical physicians and medical equipment companies. Orders can be submitted via fax or email.

Orders should include the following information:

  • Order number
  • Billing address
  • Shipping address (must be a full street address; we cannot ship to a P.O. box)
  • Product numbers and quantities
  • Contact person's name and fax number
  • Planned payment method: credit card (MasterCard or Visa) or wire transfer
  • Orders from physicians should be sent via fax and should include a copy of a written prescription showing quantity for each item. The prescription should be written on your usual prescription form showing your medical license number.

Payment Options

Please note that all international orders must be prepaid via credit card or wire transfer. Once we receive the order, we will prepare and fax to you a Proforma Invoice for confirmation. The Proforma Invoice will show all charges including shipping.

Shipping

We usually ship via UPS. Shipment via other carriers (FedEx or DHL) is possible if you provide us with your account number for direct billing. The carrier must be able to pick up the shipment from our office in Irvine, California. Please note that FDA and ISO regulations require that we ship via a method that allows the shipment to be traceable at all times.

Once we receive your confirmation of the Proforma Invoice and payment in full, your order will be shipped immediately. We will notify you of the expected delivery date and the carrier's shipment tracking number.

Please note that you will be responsible for handling any customs fees or arrangements once the shipment arrives in your country.
