The Passy Muir® Valve is a prescription device and requires an order from a medical doctor. The prescription you receive should be maintained in the patient’s file. Do not send a prescription to Passy Muir unless requested.
To Place An Order

Passy Muir accepts orders from hospitals, durable medical equipment companies, doctors’ offices, nursing homes, and pharmacies. Orders may be placed by phone, fax, or mail.
Each order must include the following information:
- Customer ID # when known
- Billing address and phone number
- Shipping address, phone number and contact name
- Product Number, Description, Price, and Quantity (Each Valve sold separately)
- Facility Type and Department requesting the Valve
- Shipping requirements (Ground or Priority)

Because the Valve is a prescribed medical device, Passy Muir cannot accept returns or exchanges.
Unless requested otherwise, orders are shipped via FedEx. Shipping options include standard, 3 day, 2 day, overnight delivery, and in some areas, Saturday overnight. Please speak with a customer service representative to learn about your shipping options. We cannot mail directly to a patient or P.O. Box.
Billing occurs at the time of shipment. Payment is due 30 days upon receipt unless you are a prepaid customer. Payment can be made via check or credit card. An account with Passy Muir is established when placing your order. Pre-approved credit is required to become a 30-day net customer. Please click here to download a credit application.
Insurance Coverage & Reimbursement
Unfortunately, we do not take any medical insurance as a form of payment. However, we carry HCPCS Prosthetic Codes for our products that you may submit to your insurance for possible coverage. Please see below for more information or give us a call if you have further questions.
- HCPC Prosthetic code is L8501 for all valves
- HCPC Prosthetic code is E1399 for all Adapters and Secure-Its.

Note: We do not take any medical insurance as a form of payment. The patient must pay out-of-pocket. However, we can provide you with a billing code for our products that you may submit to your insurance for possible coverage.
